[PATCH] a100u2w: cleanups

 - mark all functions static
 - don't use scsi.h obsolete symbols
 - add my copyright boilerplate (more for last years work then the
   trivial nitpicks here)
 - comment formatting fixes
 - remove dead wood from a100u2w.h
